Dear Patient,

As of 2/1/2016, I will be instituting a "no show" policy for my patients.  A "no show" will be defined as not being present for a scheduled appointment, cancelling an appointment within 24 hours of the scheduled time or twenty minutes late for an appointment.

A fee schedule for "no shows" will be as follows:
$20.00 for an established patient or follow up visit
$50.00 for a new patient visit
$50.00 for procedures

As always, I do understand that things "come up" and emergencies do happen.  The "no shows" will be reviewed and, with our office's discretion, be charged accordingly.  This is being done to encourage communication from my patients, attempt to reduce wait time for appointments and to streamline the office experience for everyone.

Also, if a pattern of no show appointments is seen developing, termination of that patient could be instituted.  For example, three no show appointments in a row would be grounds for dismissal of the patient.  These will be decided on a case by case nature and reviewed by our office.
